New Client Forms
Prior to your first visit you are welcome to download our new client forms:
Upon entering our office, our Chiropractic Assistant (CA) will welcome you. She will request that you complete or turn in your completed new client form. This paperwork provides us with your health history and information on your condition.
1st Visit – New Client Consultation & Examination
On your first visit you will have a consultation with one of our chiropractors to discuss your health-related problems and concerns. This initial visit is designed for our chiropractors to learn more about you, your condition, and expectations to determine how chiropractic care can help you meet your goals.
After your consultation, our chiropractors will perform a through and complete chiropractic examination. The examination will analyse your spine and nervous system integrity. The examination will include standard neurological, orthopaedic, postural, spinal analysis and physical tests.
After the examination is finished, the chiropractor may require a set of x-rays to be taken. The x-rays assist the chiropractor in determining the most effective care plan for you, and alert us to any serious spinal condition that could affect your care in the office. Many clients are amazed to see the condition of their spine and really understand the misalignment and degeneration once it has been explained to them.
At this point the chiropractor will ask you if you have any questions and concerns. Upon reviewing your case and potential X-rays, the chiropractor will determine the appropriate care plan that will suit your need and will be discussed at your Report of Findings visit.
2nd Visit – Report of Findings
Once your x-rays have been assessed and correlated with your exam findings, the chiropractor will give you a detailed report of all findings and answer any questions including:
- What is wrong?
- How long will it take?
- Your program of care
- How chiropractic care can help
After a thorough review of your health history, goals, and examining your spinal findings and X-rays, the chiropractor will discuss the key recommendations and notify you if you are fit for chiropractic care. The chiropractor will provide the best treatment and wellness program for your individual needs.
Spinal Checks and Adjustments
At the conclusion of your report of findings visit, the chiropractor will adjust your spine if warranted. If you have any concerns at any point through the adjustment process let the chiropractor know and they will modify their technique to satisfy your concerns. After your first adjustment the chiropractor will check in before your next visit to see if you have any other questions or concerns about care.
3rd Visit- CA visit and Program of Care
Upon arriving at the office you will be greeted by one of our CA’s at reception. You will be guided to an adjusting room where our office’s policies will be discussed with you. The chiropractor will outline your tailored program of care whose purpose is to help you achieve your health goals. The team at Wellspring work with our clients in a collaborative environment, our clients expect exceptional care from our chiropractor and in return we expect our clients to follow any recommended lifestyle changes. Upon agreement, your care plan will commence, your spine will be checked and adjusted if necessary. At the conclusion of the visit the CA will book your schedule of care.
Your spine will be analysed. If vertebral subluxations are found, your spine will be adjusted. How your spinal and nervous system function is progressing will be monitored and reported to you at appropriate intervals with progressive examinations and reports.
12th Visit – Progress Examination
At your progress examination, you will be asked to complete a subjective progress report. A thorough comparative examination will be conducted to see how you have progressed under care. At this point, the chiropractor will make recommendation based on their findings, your updated forms and will suggest adaptions to your program of care based on your current health goals and needs. The stage of care you are currently in will be discussed along with steps on how you can progress to the next level of care.
Annual Chiropractic Review
Every 12 months your chiropractor will review your case history, conduct a thorough comparative examination, and update your care plan as necessary based on your progresses over the previous year. At this point, further recommendations will be made to ensure that you are receiving the best care possible.
